Womenwhotakeantidepressantswhilepregnantmayberaisingtheriskoftheirchilddevelopingattentiondeficithyperactivitydisorder(ADHD)butnotautism,astudypublishedinthejournalMolecularPsychiatryhasfound.1
TheUSresearchers,however,warnedthatanypotential
“modest”riskfromtakingantidepressantsshouldbeweighedagainstthepossibleconsequencesofnottreatingmaternaldepressionoranxietyineachindividualpatient.
Todiscontinueantidepressantsduringpregnancycouldincreasetheriskofrelapsefivefold,theysaid,andmaternaldepressionduringpregnancywasassociatedwithhealthcomplicationsforthemotherandchild.
Theauthorsconcluded,“Decisionmakingaboutantidepressantutilisationinpregnancyrequiresthattheseemergingrisks,aswellasbenefits,beweighedinthecontextoftheindividualpatientandfamily.”
RoyPerlis,leadauthorandapsychiatristatMassachusettsGeneralHospital,toldTheBMJ,“Ithinkourresults,takentogetherwithsomeotherstudiesthatdidnotfindsignificantrisk,shouldbereassuringtowomenwhoneedtotakeantidepressantmedicationsduringpregnancy.
Anapparentlyrapidincreaseinprevalenceofautismspectrumdisorder(ASD)overthepastdecadehasincreasedeffortstoidentifypotentiallymodifiablerisks,althoughexpertsbelievethatmuchoftheliabilityforautismisinheritedandthat
environmentalfactors,notyetclearlyidentified,mightaddtotherisk.

Previousstudieshavehadcontradictoryfindingsaboutwhetherprenatalexposuretoserotonergicdrugsleadstoautism-likequalitiesinchildrenandwhetherinuteroexposuretoantidepressantsincreaseschildren’sautismliability.
AteamofresearchersfromthedepartmentofpsychiatryatMassachusettsGeneralHospitalinBoston,USA,thereforelookedintothesubjectinmoredepthtoseeifotherfactorsmightbeinvolved.Theystudiedassociationsbetweenprenatalantidepressantexposuresandtheneurodevelopmentaldisordersin1377childrenwithautism,2243withADHD,andamatchedgroupofchildrenwithoutthosedisorders,drawnfromthemedicalrecordsofalargehealthsystem.
“Iwouldalwaysadvisecautioninprescribingantidepressants;butweneedtokeepinmindthat,inmanycases,thebenefitsfaroutweightherisks.Acrucialpointisthatnotadequatelytreatingdepressionandanxietyinpregnancycanalsocarrysubstantialrisk.”
Theauthorsfoundthatantidepressantexposureduringpregnancywasassociatedwiththeriskofautism,buttheseverityofmaternalmentalillnessalsoseemedtoaffectthisrisk,andtheassociationwasnolongersignificantwhentheytookintoaccountthemother’shistoryofmajordepression.Conversely,therateofADHDwasslightlyincreasedinchildrenwhowereexposedtoantidepressantsprenatally,evenafteradjustingfortheseverityofmaternaldepression.
Overall,theresultsshowedthattheriskofautismpreviouslyobservedwithprenatalantidepressantexposuremightbebetterexplainedbytheriskassociatedwithmaternalillness.However,theyalsoshowedthatsuchexposurecouldbemodestlyassociatedwiththeriskofADHD.
Expertsreactedtothestudywithcaution.CelsoArango,
scientificdirectoroftheSpanishCentreforBiomedicalResearchinMentalHealthbasedinMadrid,said,“Fromthechild’sperspectiveitislikelythatthepotentialharmcausedbyanyincreasedriskofADHDorautismwouldbemuchlessthanthepotentialharmofhavingamother[with]depression.Thisresearchdoesnotmeanthatwomenshouldstoptakingtheirantidepressants.”
GuyGoodwin,presidentoftheEuropeanCollegeof
Neuropsychopharmacology,said,“Itremainspossible,evenlikely,thattheeffectobservedisduetoincreasedgeneticrisksofpsychiatricdisorderintheoffspringofthewomenprescribedantidepressants,ratherthantheeffectsofthe
drugs.”
TheauthorssaidthatcliniciansshouldbecarefulaboutchangingtheirprescribingpracticesoradviceuntilfurtherstudieshadbeencarriedouttogetamorepreciseestimateoftheeffectsofantidepressantsonADHDrisk.Theyadded,“Theconsequencesoffailingtotreatdepressioninpregnancywarrantemphasis.”
