《授权委托书》填表说明
1、申请单位到市食品药品监督局办理行政许可事项时，申请人不是法定代表人或负责人时，须由法定代表人或负责人开具《授权委托书》给申请人，列明授权范围和委托期限。
申请人凭《授权委托书》到市食品药品监督局受理大厅办理相关事宜。
2、“委托人”，填写法定代表人或负责人姓名。
“被委托人”，填写具体办事人员姓名。
“工作单位”、“职务”、“联系电话”，据实填写。

8、代为办理该房产《土地使用证》的更名手续并签署相关文件;
9、代为到当地税务部门交纳该房产交易时所产生的各种税费并在契税、营业税、个税、印花税等税费单上签字确认，同时办理、领取该房产的《契证》;
10、代为办理(或放弃)该房产二手房资金监管的一切手续并签署相关法律文件;
11、代为办理该房产《房屋所有权证》、《契证》、《土地使用证》的挂失、补办手续;
12、代为办理该房产室内的水、电、煤气、有线(数字)电视、宽带网、电话、物业、采暖等配套设施更名手续;
13、代为协助买房人办理该房产的银行交易贷款(或住房公积金贷款)手续，并协助买房人到房产抵押部门(或担保部门)办理该房产的抵押(担保)手续、领取他项权利证及签署相关法律文件;
14、代交各种费用、代为开立银行放款账户、代为收取银行放款、代为收取售房款;
15、上述房产如遇动迁，受托人有权代替委托人办理该房产的动迁(回迁)手续及相关事宜、代为签署《拆迁协议》、代为开立银行账户并领取动迁补偿款。
16、代为到房产部门办理该房产非金融抵押的一切手续并签署相关文件;
17、有关上述房产出售的未尽事宜，受托人可以全权处理。
本项授权为我们二人真实意思表示，受托人在办理上述委托事项期间所签署的一切法律文件我们均予同意，其引发产生的相应权利和义务由我们承担。

【授权委托书公证相关问题】
法人授权委托书公证，是指公证机关根据法律规定和当事人的申请，对法定代表人出具的授权委托书的真实性、合法性予以证明的活动。
申办法人授权委托书公证，当事人应向法人住所地公证处提出申请。
一、申请办理法人授权委托书公证，当事人应向公证处提交以下证件和材料：
法人资格证明;
法定代表人资格证明及其本人的身份证件;
拟办理公证的授权委托书(公证处亦可代拟授权委托书)。
二、办理法人授权委托书公证应注意以下问题：
法定代表人应亲自到公证处提出申请，不得委托他人代理。
授权委托书应当载明代理人的姓名或者名称、代理事项、权限、代理日期和有效期、有无转委托的权利等，授权委托书应由法定代表人在公证员面前亲自签名、盖章，并加盖法人公章。
